I am the custodial parent or legal guardian of the subscriber on this policy and have the legal authority to make health care decisions on behalf of the subscriber.
There are no legal restrictions, including but not limited to court orders, which limit my ability to access the subscriber’s information.
I am creating this account only to manage my financial responsibility for the subscriber’s health insurance policy.
When the subscriber turns 18 years old, the subscriber has a legal right to privacy of his or her health information. At that time, I understand that I will no longer be permitted by law to access the subscriber’s account, despite any ongoing financial responsibility that I have for the health insurance policy. The subscriber’s My Health Plan account will reset automatically and will require the subscriber to re-register.
Medical Mutual reserves the right to request documentation as proof of my custodial parental or legal guardian rights at any time and I understand that I must provide that documentation within 24 hours or my access to the subscriber’s My Health Plan account may be suspended until the appropriate documentation is provided and verified by Medical Mutual.
I understand that if I don’t comply with these Terms and Conditions, Medical Mutual may disable my access to the subscriber’s My Health Plan account immediately.
To reset your password, you must request a confirmation code, which we will send to the email address on file. Once you receive the email, enter your confirmation code in the box below and click Next to finish resetting your password.
